Community Health Workers, Promotores, and Representatives (CHW/P/R) are trusted members of their communities who bridge gaps between systems and underserved populations. They deliver culturally responsive care, address social determinants of health, and advocate for equity.
The Los Angeles County CHW/P/R Consortium unites CHWs, community-based organizations (CBOs), and public health leaders to strengthen collaborative efforts. Through advocacy, training, and sustainable funding models, the consortium empowers CHWs to improve health outcomes and foster equity across Los Angeles County.

To foster collaboration among CHWs, Promotores, Representatives, and their organizations, building capacity and amplifying their impact on health equity, community empowerment, and systemic transformation.

Transforming Health Equity
The Los Angeles County Community Health Workers, Promotores, and Representatives (CHW/P/R) Consortium was established in 2024 as a transformative initiative to address systemic health disparities across Los Angeles County. Building on the foundational efforts of the Alliance for Health Integration Assessment (2022-2023) and the One-Year Action Plan for the Community Health Worker Workforce (2023–2024), the Consortium brings together Community-Based Organizations (CBOs), CHWs, and public health leaders to amplify community-driven care and equity initiatives.
Guided by a vision of creating resilient, healthy communities, the Consortium has achieved key milestones in its journey. We are in the process of uniting 150 CBO’s and health partners to lead our strategic actions. We have launched strategic workgroups focused on data integration, policy advocacy, and workforce development, and will introduce innovative funding models to sustain CHW/P/R programs. These efforts will improve access to culturally tailored care, enhance training opportunities for CHW/P/Rs, and foster stronger partnerships among members.
The Consortium’s impact extends beyond immediate health outcomes; it will empower underserved communities by addressing the root causes of health disparities, fostering advocacy, and championing systemic change. As a backbone organization, El Sol Neighborhood Educational Center has played a pivotal role in aligning stakeholders, driving strategic planning, and ensuring the sustainability of these initiatives.
Together, the Consortium and its partners are transforming the healthcare ecosystem in Los Angeles County, elevating the role of CHW/P/Rs, and creating a model for community-centered care that prioritizes equity, compassion, and collaboration.
