The Los Angeles County CHW/P/R Consortium thrives through the strength of its partnerships. By uniting Community Health Workers, Promotores, and Representatives (CHW/P/Rs) with Community-Based Organizations (CBOs), government agencies, and strategic partners, we create a powerful network committed to advancing health equity and addressing systemic disparities.

Our partners are at the heart of every initiative, contributing their expertise, resources, and shared vision to build healthier, more resilient communities across Los Angeles County. Together, we amplify the voices of CHWs/P/Rs, foster impactful collaboration, and deliver sustainable solutions that transform lives.

Types of Partners

Community Health Workers, Promotores, and Representatives (CHW/P/Rs)

At the heart of the consortium, CHW/P/Rs are trusted community leaders who provide culturally sensitive care, advocate for equity, and connect individuals to vital services. Their lived experience and community connections make them indispensable in addressing health disparities and fostering systemic change.

County and Institutional Partners

Public health departments (e.g., DPH, DMH, DHS) and other institutions collaborate with the consortium to align resources and strategies, advocate for systemic policies, and provide technical support to expand CHW/P/R programs.

Community Advocates and Additional Partners

Non-traditional collaborators, including schools, housing agencies, and parent advocacy groups, bring unique perspectives and resources to address broader social determinants of health. These partnerships enable holistic and comprehensive community care.

Community-Based Organizations (CBOs)

CBOs employ and support CHW/P/Rs, providing the infrastructure needed for effective, community-centered care. They partner with the consortium to deliver programs, tackle social determinants of health, and strengthen local capacity.

Funders

Philanthropic organizations, private donors, and corporate sponsors are vital to the consortium’s sustainability. These partners provide funding for capacity-building efforts, innovative programs, and systemic change initiatives that strengthen CHW/P/R workforce and infrastructure.

Advisory Group

This multi-sectoral group of CHW/P/R leaders, CBO representatives, and public health officials provides strategic direction, ensures alignment with the consortium’s mission, and helps drive impactful initiatives.

Strategic Partners

Academic institutions, research organizations, and technology providers offer expertise, innovative solutions, and evidence-based approaches to enhance the consortium’s work. These partners help integrate new tools, technologies, and training systems.

These strategic partners and advisors supported and informed the formation of the Consortium and helped to create the appropriate infrastructure and relationship-building to ensure successful planning, engagement, and launch. Additionally, strategic partners and advisors participated in 7 collaborative planning sessions to inform and guide our 4-year strategic plan. .
